What Is Ecosystem Mapping in Patient Engagement?
Published: July 7, 2026 | Category: Commercial Strategy | Author: Liza Prettypaul-Lodhia
If you ask ten pharma leaders where their patients are actually dropping off, you'll get ten different answers. Not because they don't know their programs. Because each of them is looking at a different piece of the same patient journey.
That's the gap ecosystem mapping is built to close.
This post defines what ecosystem mapping actually is in patient engagement infrastructure — and why it's different from patient journey mapping, vendor performance mapping, and process mapping. It explores what ecosystem mapping consistently reveals: points of friction where patients quietly drop off, definition mismatches across functions, and infrastructure gaps where no vendor was hired to cover.
Each finding ties to a specific strategic problem senior leaders are trying to solve — from explaining persistence declines to boards, to aligning cross-functional accountability, to prioritizing where to invest first based on revenue impact.

A CCO at a specialty pharma company said something at a dinner recently that's stayed with me. "We're hitting all our activation metrics. Enrollment is up. First fill is strong. But our two-year persistence keeps eroding. And nobody at the board table wants to hear that we don't know why."
Strong activation. Declining persistence. No clear connection between the two.
It's not an operational problem. It's a revenue problem.
This post explores why most pharma is investing in the wrong part of the patient journey. The first 90 days concentrate budget on activation. The next 18-24 months — where therapeutic continuity is actually decided — gets what's left over.
But the revenue math runs in the opposite direction. Year 2 and Year 3 persistence is where lifetime value compounds. Small improvements there produce outsized returns. The senior commercial leaders who recognize this aren't asking how to improve patient engagement anymore.

Published: June 23, 2026 | Category: Life Science | Author: Linked Patient Learning
Most patient engagement programs don't fail at launch. They lose momentum somewhere between month 3 and month 9 — and by the time persistence data confirms it, the damage is already done.
Strong enrollment. Successful first fill. Declining refills by month 6. By month 12, you're explaining persistence drops to your board with answers that don't satisfy anyone.
The pattern is consistent across pharma. Across therapy areas. Across company sizes.
This post explores why patient engagement programs lose momentum invisibly — and what most pharma teams don't have in place to catch it before persistence data confirms it. It identifies the four root causes hiding in plain sight: cross-functional fragmentation, touchpoint optimization without continuity tracking, reactive infrastructure, and misaligned definitions.
By the time momentum loss is visible in your dashboards, it's been building for months. Here's how to see it earlier.

A patient services VP showed me her dashboard recently. Enrollment 95%. Benefit verification 48 hours. First fill rate 87%.
Then she showed me her persistence data. Down 18% year-over-year.
"I can't explain it," she said. "Every metric we measure says we're doing things right."
That's the gap I keep seeing. Strong metrics. Declining outcomes. No clear connection between the two.
This post introduces the 90/10 Framework — the framing I use to explain what most pharma teams can't see. The 10% is what your infrastructure was built to measure: activation moments. The 90% is everything else: the daily decisions, coverage transitions, and handoff failures that determine whether activation becomes outcomes.
Most pharma optimizes the 10% relentlessly. Persistence is won or lost in the 90%. Here's why it's so hard to see — and what naming it makes possible.

When a patient engagement gap crosses functional lines, who owns it?
I've started asking pharma leaders this question — Medical Affairs, Commercial, Patient Services, Brand. Different roles. Different companies. Different therapeutic areas. The answer is almost always the same: "It depends."
Then a pause. Then a list of who might own it. Then qualifications about who actually does. Then a quiet acknowledgment that it's nobody's clear responsibility.
This post examines the cross-functional ownership problem in patient engagement infrastructure — where the gaps that matter most live between functions, and the cost compounds quietly across hundreds of patients per year. It explores what shared visibility actually requires across Medical Affairs, Commercial, and Patient Services, and offers three concrete elements that distinguish companies starting to solve this problem.
The first step isn't fixing the gap. It's making it visible.

A patient services VP told me last month: "I know our infrastructure has gaps. I've read every framework. I understand the 90/10 problem. I just haven't done anything about it."
She's not alone.
I've had this same conversation—word for word—with four different pharma leaders this quarter. Smart people. Senior roles. Real budgets. Genuine commitment to patients.
All stuck in the same place: between understanding the problem and acting on it.
This post explores the pattern emerging from conversations with pharma decision makers across commercial leadership, patient services, analytics, and brand teams. They weren't lacking resources, authority, or conviction. They were lacking certainty that change would work for them before they tried it.
What changed for one of them? She mapped one patient journey. Just one. Two hours. Found a 12-day gap where 23% of patients were dropping out. Sometimes the framework only becomes real when you see your own data.

Rare disease companies invest heavily in patient engagement—dedicated teams, hub services, specialty pharmacy partnerships, foundation coordination, field reimbursement support. The investment is there. The commitment is real.
But here's the problem: patient engagement infrastructure follows an industry model designed around chronic disease assumptions—large, renewable patient populations with lower lifetime value.
In rare disease, the economics are fundamentally different. Patient populations: 200 to 2,000 globally. Finite. Patient lifetime value: $500K to $2M+, sometimes $6M+. Therapy duration: life-long.
This article examines why infrastructure built for chronic disease flow destroys rare disease preservation economics. It explores provider hesitation dynamics (specialists with only 8 patients total), patient community impact (ultra-rare patients know each other—bad experiences ripple), and the three infrastructure must-haves for protecting irreplaceable $2M assets: cross-ecosystem visibility, proactive coverage monitoring, and early intervention protocols. Because in rare disease, infrastructure designed for renewable populations will destroy finite ones.

You understand the 90/10 Framework. The 10% is activation—enrollment, benefit verification, first fill. The 90% is sustained support—daily decisions, coverage transitions, handoff coordination. Your infrastructure was built for the 10%. Patients discontinue in the 90%.
You've seen the evidence: board meetings where you can't explain declining persistence, payer negotiations where you lack outcomes data, vendor dashboards showing excellent metrics while patients disappear in gaps.
You get it. So why haven't you started?
This article examines the real obstacles preventing pharma decision makers from building 90% infrastructure: the belief it's a massive project, vendor lock-in concerns, buy-in requirements, and complexity paralysis. More importantly, it reveals what these obstacles actually represent—uncertainty about whether the framework will work—and why the companies building proof right now aren't more certain than you. They just decided proof matters more than certainty.

Published: May 6, 2026 | Category: Life Science | Author: Linked Patient Learning
Your patient engagement platform dashboards show 95% enrollment completion, excellent call resolution, and strong satisfaction scores. But your persistence is still declining.
The problem: Your healthcare engagement solutions measure vendor performance, not patient outcomes. Before you auto-renew contracts, you need to understand what your vendors CAN deliver versus what infrastructure you need to build yourself.
This vendor evaluation framework provides 12 due-diligence questions that assess measurement and analytics capabilities, cross-handoff data visibility, and proactive intervention systems. Each question reveals specific vendor capabilities beyond standard program performance metrics.
The framework includes a scoring system (8-12, 4-7, or 0-3 points) with clear decision pathways: optimize contract terms for high-scoring vendors, build supplemental infrastructure for mid-range scores, or implement ecosystem-level solutions for vendors lacking cross-vendor visibility.
Most vendors score 2-4. That's not failure—they weren't designed for cross-ecosystem orchestration. This framework helps you make the right renewal decision.

Your quarterly board meeting is coming. Last quarter, they asked why persistence is declining. You said "patient non-adherence." They weren't satisfied.
Next quarter, you need better data—not new vendors or 18-month infrastructure projects. Data you can get starting this week.
This article presents three tactical quick wins that prove the 90/10 Framework business case and deliver board-ready answers: Map one high-risk patient journey to reveal handoff failures. Add root cause tracking to discontinuation data so you can explain WHY patients leave, not just that they did. Create coverage gap early warning for your top 100 patients to test proactive intervention.
Each uses existing systems, delivers value in 30-90 days, and builds toward full 90% infrastructure. All three give you board-ready answers for next quarter—then reveal why manual approaches don't scale and when you need automated infrastructure.

Published: April 8, 2026 | Category: Life Science | Author: Linked Patient Learning
Patient support dashboards typically track operational metrics: enrollment rates, PA turnaround times, call resolution, touchpoint completion. These measure vendor performance in the 10% (activation moments) but miss what's happening in the 90% (patients' daily lives).
When coverage churn causes discontinuation, it appears in your data as "non-adherence" or "patient choice"—not as the coverage disruption it actually is. With the One Big Beautiful Bill Act driving repeated Medicaid redeterminations and 10 million people losing coverage by 2034, this metrics blindspot becomes critical.
This article introduces 3 core metrics that patient support teams should track instead: Coverage Gap Detection Time, Discontinuation Root Cause Attribution, and Coverage Disruption Rate. These metrics reveal why patients discontinue—not just that they did—enabling you to explain declining persistence to your board and build infrastructure that maintains therapeutic continuity through coverage volatility.
